I erased the colour inside his eyes first of all. Then I created a new layer underneath and added a simple red rectangle which then appeared through the erased eye-sockets. I did some drop-shadow on the eye sockets so it blended a little better and finally applied a slight hue to the red rectangle so the red wasn't too strong
